How Do I Become an FSA member?

The Florida Society of Anesthesiologists (FSA) is a state component society of the American Society of Anesthesioloigsts (ASA). FSA members must also belong to ASA, and the FSA uses the ASA membership application for state membership.

Go to http://www.asahq.org/joinASA.htm to choose the appropriate member category and application.

Florida applicants must submit a completed ASA application, as well as a copy of their Anesthesiology Board certification and a copy of their current Florida medical license.

FSA has the following available categories for membership:

  • Active (practicing anesthesiologists in the State of Florida) - annual dues $395
  • Affiliate (anesthesiologists in service of U.S. Government, including V.A.; research scientist interested in anesthesiology physician not in clinical practice of anesthesiology) - annual dues $75
  • Resident (physician in full-time training in Anesthesiology residency in Florida) - annual dues $5
  • Educational (anesthesiologist assistants [AA]; Certified Registered Nurse Anesthetists [CRNA]) - annual dues $100
  • Educational Student (AA students; CRNA students) - annual dues $25

Current Members - How do I renew?

You may renew your FSA membership by paying your dues online via PayPal. Click here to pay your dues if you are a renewing member.

Alternatively, you may call 800/400-3211 and pay by credit card (MasterCard, Visa or American Express). You may also mail your payment to FSA, PO Box 13978, Tallahassee, FL 32317, or fax it to 850-656-3038.

 

Educational Membership Category & Update for Affiliate members!

Educational Membership now available!

Practicing AAs and CRNAs are now eligible for FSA membership in the Educational category, and AA and CRNA students are eligible for the Educational Student category.As with all FSA membership categories, Educational & Educational Student members are required to attain and maintain ASA membership. FSA uses the ASA educational membership application, which can be found by clicking here. FSA Educational dues are $100 per calendar year and Educational Student dues are $25 per calendar year.

Dues waivers now available for DEPLOYED FSA Affiliate members!

Current Affiliate members deployed/on active duty in the U.S. Armed Forces may request dues waivers on a year to year basis.
