GUIDE TO CLAIMING CME CREDIT FROM THE 2011 FSA ANNUAL MEETING

Here's what to do to claim your CME credit. It's as easy as 1, 2, 3...

  1. Click the link above to access the evaluation system (or click here)
  2. Log into the system using your unique attendee ID number as your username.
    • Your attendee number was emailed to you before and after the meeting, and was also printed on a label under your name on your registration packet.
    • Your password is "fsa" (all lower case). Each attendee does not need a unique password as only registered attendees with a unique attendee ID number will be able to access the evaluation system. 
  3. Complete the electronic evaluation for each course attended. Then, simply save or print your certificate. 

REMEMBER: Attendees are ethically bound to claim only the credit for the education they physically attend. All evaluations must be completed electronically no later than June 23, 2011 in order to comply with our accreditation requirements.

This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education through the joint sponsorship of the Florida Medical Association and the Florida Society of Anesthesiologists.

The Florida Medical Association is accredited by the Accreditation Council for Continuing Medical Education to provide continuing medical education for physicians.

The Florida Medical Association designates this live activity for a maximum of 23.0 AMA PRA Category 1 Creditsâ„¢. Due to concurrent sessions, 16.0 credits is the maximum number of credits one participant may earn.

Physicians should claim only the credit commensurate with the extent of their participation in the activity.

FSA 2012 Annual Meeting
June 8-10, 2012
The Breakers, Palm Beach, Florida

Meeting registration is scheduled to be available no later than the end of 2011.  CME approval will be obtained in early 2012. 

Hotel reservations for the 2012 annual meeting will be open in July 2011. Be sure to reserve a hotel room for Thursday, June 7, 2012 as we plan to offer lots of great activities and education on Friday, June 8! You won't want to miss it. Reservations go quickly at the Breakers and we want our attendees to get their first choice of room type.
